To get a card immediately, print off the application form fill it out and take it into your nearest Duane Reade location. Application forms are also available in any of our Duane Reade locations.

Duane Reade will not sell or give specific information about you to manufacturers or direct marketers. As an added value to our customers, Duane Reade may send customized offers and information based on your purchases. To do this we may use outside agents to help process and send these offers. These agents are bound to strict confidentiality. If you do not wish to receive offers, information and mailings from Duane Reade, please check the appropriate box on the application form.
