If you have filled your prescription at a Duane Reade Pharmacy and would like to access your patient profile, be sure to include the following information in the Prescription Information - Optional section on the registration form:
- Prescription Number
- Store Number
- Date of Birth
When entering "Home Phone Number" on the Registration Page, be sure to use the number you provided to the Pharmacy. The Home Phone Number on file at the Pharmacy can be found on your prescription receipt.
